If permission is granted to attend the Liberty Elementary School, I will accept the responsibility for transporting
my child to and from school each day. I understand that continued enrollment in the Liberty Elementary School District
No. 25 will be contingent upon the student’s prompt and regular attendance, adherence to District policies, individual
school rules, and to the items listed in the Open Enrollment Parent/Student Agreement. I understand the effort and
achievement toward educational objectives may be a factor in the decision regarding permission to attend Liberty
Elementary School District No. 25. Open enrollment transfers must be applied for annually. A student who has been
expelled by any school district in this state or who is not in compliance with a condition of disciplinary action
imposed by any other school or school district or with a condition imposed by the juvenile court shall not be admitted.
Acceptance for enrollment may be revoked upon finding the existence of any of these conditions; or, if falsification of
information is confirmed. Transportation is the responsibility of the parent/guardian. The Liberty School District will
not transport students attending under Open Enrollment for any reason. I agree that the information entered on this
application is true and accurate, and I accept the information noted above.
